Job Description
Will perform diversified administrative support duties for the transplant program director and the transplant nursing staff. Functions as an integral part of the transplant team, ensuring that proper assistance is provided to nursing staff so the patients are moved through the different phases of transplant care efficiently and effectively.
The Transplant Assistant for Transplant supports the Transplant Program both internally and externally of the organization. The primary responsibility is to perform non-nursing tasks that are peripherally related to the job duties of the transplant coordinator. In transplantation, many diagnostic tests and consultations are ordered from the clinic as part of the transplant work-up for evaluation. The Transplant Assistant is integrally involved in executing this process.
Additionally, the Transplant Assistant supports the Transplant Social Workers and Financial Coordinator in areas regarding pre-transplant evaluation. The Transplant Assistant reports to the Director and Manager of Renal Transplant Center, and is under the direct supervision of the Pre-transplant Coordinator.
This position must communicate effectively and professionally with the transplant team, physicians, surgeons, patients, families, and other departments.

ESSENTIAL JOB RESONSIBILITIES:

Conducts intakes/inquiries for potential transplant recipients and living donors.
· This involves capturing demographic information and performing a basic telephone medical screen.
· Create patient charts and request medical and insurance records from the dialysis units
· Delivers pertinent information to Transplant Social Worker and Financial Coordinator in order to continue the evaluation process. This is documented in the transplant database.

Schedules for the bi-monthly transplant orientation class
· Schedules patients to attend class
· Schedules transplant history and physical appointment with Nephrologists and Surgeons
· Prepare patient pre-transplant charts with documentation
· Prepares pre-transplant patient handbook and supplemental information
· Works in coordination with Pre-Transplant Coordinator to prepare orders for patient testing
· Communicates to patient registration regarding patient schedules

Schedules evaluation and re-evaluation testing appointments for pre-transplant patients.
· Assists Pre-Transplant coordinators with the required tests which include diagnostic tests and physician consultations
· Assists Pre-Transplant Coordinators and Transplant Data Coordinator on capturing charges for the ordered procedures in the Transplant Database.
· Communicates to patients confirming appointments both verbally and in written format.
· Follows up with hospital and physician offices to gather results.
· Collects all diagnostic test results for chart
· Communicates the results to Pre-Transplant Coordinators

Communication with patients, physicians, dialysis units, hospital and Transplant Team
· At referral, financial clearance, orientation class and beyond, the Scheduling Coordinator must utilize the Transplant Database to capture all pertinent information.
· Upon referral, sends letters to patients, physician and dialysis unit acknowledging referral and providing a guide to the patient.
· Upon financial clearance, call made to patient acknowledging clearance, informs patient and schedules patient for orientation and initial follow up as indicated.
· Communicates pre-testing appointments to patients via letter and phone
· Upon listing, sends letters to patients, physician and dialysis unit (if applicable)
· Works with Transplant Social Workers and Pre-Transplant Coordinators to address concerns with patient compliance via database and phone.

Assists Nurse in preparation for clinic
o Conducts chart preparation.
o Patient room and take vitals.
o Fills out the clinic form, CDC and PBT high risk forms and Medical Release paperwork with patients and enters pertinent data in the transplant database.
o Enters vital signs in the transplant database.
o Review the medication sheet with patient before seeing physician.
o Cleans rooms.
o Assist patients with mobility issues with discharge from clinic.

Must be able to perform duties independently and meet deadlines as directed by transplant coordinator or management as indicated above.
Must be able to complete assignments independently and to be able to work as part of a team; to take instructions from supervisors, to exercise problem-solving skills and to interact with all customers, including coworkers, in a professional and pleasant manner.
Displays punctuality and meets attendance requirements of the department/organization.
Assists in the development and/or update of policies and procedures for the Transplant Program.

Other duties as assigned.

Level of Experience, Certifications, Specific Skills Required
1. Administrative/front office work
2. High school diploma
3. Ability to use a computer, phone, and email

EDUCATION AND EXPERIENCE:
· Required:
High school diploma or equivalent; some college or 3 years of clerical experience.
Strong interpersonal skills required; must be able to handle multiple tasks

· Preferred:
Knowledge of medical terminology, computer skills and medical experience.
