Required: BLS or CPR from AHA only
Preferred: ACLS, PALS and or PEARS from AHA
Experience: Med Surg 1+ Yrs.
Qualifications:
Education: Must be a graduate of an accredited school of nursing.
Licensure: Licensed to practice as a LPN in the state of Oklahoma.
Certifications: Must have current AHA CPR.
Licensed to practice as an LPN in the state of Oklahoma
Must have EPIC experience.

Approved Assignment Details:

Flexible contract length - 8-26 weeks
Flexible Guaranteed Hours - 36 or 48 hours
Blocked Scheduling to include every other weekend or every weekend
RTO of 5 days or less, must make up the week on the back end
Holiday RTO ? approved based on availability

General Comments
Overview Carries out routine patient care assignments, works in controlled patient care situations, performs established nursing care procedures, and engages in therapeutic, rehabilitative, and preventive activities, under the direction of a registered nurse (RN). The licensed practical nurse (LPN) is trained to perform multiple skill functions pertinent to their clinical area. This position provides care to patients ranging in age from infant to geriatric. The LPN will perform all duties in accordance with the mission, vision, and values of Client Hospital. Makes decisions based on standard practice, instructions, operating directives and established procedures. May be required with orientation to provide service and/or patient care to other areas of Client Hospital Ardmore. Responsibilities Collaborates in the assessment of patient/significant other in a timely, consistent and systematic manner.
Performs patient assessments as indicated by the patient?s condition or as directed by the RN. Notifies physician of admission and obtains admitting orders per departmental policy.
Gathers data to be used in assessments for each patient every 8 hours or more frequently as indicated by the patient?s clinical condition and/or as directed by the RN. Includes information obtained from the patient/significant others in data gathering process.
Gathers data regarding needs and learning abilities of patient and/or significant others during admission and thereafter.
Assists the RN with identification of patient problems. Collaborates in the development of an outcome oriented plan of care that reflects the patient?s actual or potential health problems.
Collaborates with the RN in the development of the patient?s plan of care. Plan of care should include identified problems, and/or nursing diagnosis, nursing interventions, and goals/outcomes with target date.
Reviews the plan of care for assigned patients every 8 hours; collaborating with the RN in revisions as necessary. Involves the patient and/or significant others in decisions regarding care.
Prioritizes care delivery according to the patients? needs and unit activities.
Demonstrates the ability to interpret information in terms understandable to the adolescent, adult and geriatric patient.
Assists in planning care based on knowledge of growth and developmental changes occurring across the patient?s life-span. Implements the plan of care under the direction of the RN to achieve the identified outcomes for the patient according to policy and procedure.
Provides holistic nursing care to an assigned group of patients according to the prescribed nursing plan of care under the direction of the RN.
Assists with and/or performs procedures.
Provides education based on addressed needs including the safe use of equipment used in treatment or health maintenance.
Demonstrates ability to provide care and information based upon physical, psychosocial, cultural, safety and other are related factors for clients.
Demonstrates knowledge of medications and their correct administration based on the age of the patient and his/her clinical condition.
Follows the five medication rights and reduces the potential for medication errors. Assists in evaluating the effectiveness of services provided.
Collaborates with the RN in evaluating patient/significant others? responses to care and in determining progress towards achievement of outcomes.
Collaborates with the RN in evaluating patient/significant others? understanding of information.
Collaborates with the RN in evaluating the effectiveness of discharge plans for meeting the patient?s needs for continuing care.
Collaborates with the RN in obtaining data on patient satisfaction. Demonstrates effective and accurate verbal, written and electronic communication.
Creates a positive climate for communications.
Gets along well with co-workers.
Functions as a team player.
Accepts constructive criticism from supervisor.
Maintains the confidentiality of patient, staff and hospital information (written, verbal, electronic sources).
Communication is timely, accurate, concise, complete and appropriate.
Demonstrates skilled use of the telephone, Meditech and the intranet.
Utilizes constructive problem solving techniques to promote change and address issues, concerns and problems.
Completes documentation of care and other pertinent data according to organizational policies and procedures.
Communicates significant changes in patient condition and/or medical orders to the RN in a timely manner.
Maintains relevant knowledge of hospital forms. Completes patient care documentation forms and employee/patient incident reports in a timely and accurate manner.
Treats patients/family members and physicians/staff with respect, maintaining a positive influence and work environment at all times. Participates in unit and organizational activities.
Participates in 75% of staff meetings.
Participates in Performance Improvement activities such as data collection, inservices, meetings, etc.
Participates in a hospital committee, special project or service (i.e., CPR instructor, preceptor to unlicensed personnel) as requested. Complies with organizational policies and procedures.
Maintains current certification in CPR.
Attends all mandatory inservices.
Complies with all organizational policies, procedures and protocols and has no documented violations. Reports to work on time and as scheduled.
Adheres to dress code; appearance is neat and clean.
Accurately completes clock in transactions.
Adheres to OSHA standards regarding appropriate safety regulations and procedures.
Completes required competencies on a yearly basis.
Maintains hospital property and the hospital environment in a safe, clean and intact condition.
Nursing charts open by mid-shift and nursing documentation completed by end of shift, exceptions are limited. Demonstrates continued development of knowledge and skills. Uses current knowledge achieved through educational activities to enhance or improve clinical practice or patient care.
Participates in unit development and decisions making through such activities as attending staff meetings, serving on a committee or task force and/or special project work. Demonstrates the knowledge and skills necessary to provide care based upon the physical, psychosocial, educational, safety, and other related criteria appropriate to the following age groups: Neonatal, Pediatric, Adolescent, Adult and Geriatric.
Utilizes knowledge of growth and development in all patient care activities.
Includes age specific information when gathering data.
Interprets age specific data to enhance care of the patient.
Provides age specific data to other care givers.
Interprets age specific responses to therapies and treatments.
Involves families and/or significant other in decision making related to plan of care. Assists with answering call lights and following through with request. If request is not within scope of practice, then notifies the RN at the time of the request. Other: Successful completion of hospital approved medication test. Demonstrates competence by successful completion of orientation competency assessment and verification form and department specific orientation within first three months of employment. Demonstrates professional conduct and development. Able to learn and react quickly in the performance of multiple tasks. Competent to make certain independent decisions within realm of licensure and seeks assistance as required. The LPN will function autonomously, but must realize that the Registered Nurse is responsible for the overall plan of are. The LPN follows the direction of the RN and the unit/hospital guidelines. Effective interpersonal skills are required for interfacing with patients, staff, physicians and visitors. Must be able to engage in technical conversation involving discussion with physicians, nurses, patients, families/significant others and co-workers.
